RefractiveQuest – questionnaire before your consultation

Before your consultation on laser eye surgery, implantable lenses or lens exchange: a short questionnaire about your eyes, your daily life and your wishes. About 10 minutes, nothing is stored.

Dr. Valery Vinzent Wittwer

Before your consultation on laser eye surgery, implantable lenses or lens exchange, we ask you to complete a short questionnaire. This allows us to focus the consultation on your eyes, your daily life and your wishes.

Open the questionnaire

What we ask

  • About you: name, date of birth, occupation and hobbies
  • Your current glasses or contact lenses
  • Vision in daily life and at night (from age 40)
  • Eye health, previous eye operations and medication
  • Near vision and your wishes for the result

Only the questions that apply to you are shown. It takes about 10 minutes, and you can change your answers at any time.

At home or at the clinic

  • At home: at the end you receive a QR code. Show it at reception at your appointment or take a screenshot of it.
  • At the clinic: you complete the questionnaire on a tablet and hand it in at reception afterwards.

Data protection

Your answers are neither stored nor transmitted. They are only contained in the QR code or on the tablet. Your name and date of birth are used to match your answers safely to your measurements.

Please bring to your appointment

  • Your current glasses and, if available, your latest spectacle prescription
  • Records of previous eye operations
  • Stop wearing contact lenses before the measurements: soft lenses for at least 7 days, rigid lenses for at least 14 days
